ADHD is the abbreviation for Attention Deficit Hyperactivity Disorder. ADHD is one of the most common childhood behavior disorders. Of all children referred to mental health professionals about 35% are referred for ADHD, more than for any other condition. Those with ADHD often have problems in most areas of their life, including life at home, at school, at their job, and in their relationships.

"Attention Deficit Hyperactivity Disorder" is a neurological disorder that impacts somewhere between 5% and 10% of people, young and old. ADHD impacts four main categories:

Attention - causing people to have problems paying attention and focusing on a task, or finishing tasks, especially if they are not very interesting tasks.

Impulsivity - causing a lack of self-control. Impulsive behaviors, or impulsive choices, can cause havoc in relationships, work, school, or life.

Hyperactivity - Many (though not all) with ADHD are "bouncy" and hyperactive, always "on the go" and restless. About 1/2 of all who are diagnosed with ADHD are hyperactive, and the other 1/2 are not.

Easily Bored - Unless the task is very stimulating, like a video game or TV program, or playing outside with friends, those with attention disorders are often easily bored by a task - especially bored by homework, math tests, balancing checkbooks, or doing taxes, and many of these tasks just never get done.

ADHD: Attention Deficit Hyperactivity Disorder

Researchers from the National Institute of Mental Health, part of the National Institutes of Health, talk about the symptoms of ADHD as well as the latest research.

ADHD Impact

Children, teens, and adults can all suffer from ADHD. "Attention Deficit Hyperactivity Disorder" impacts somewhere between 5 percent and 10 percent of children and teenagers, and about 5 percent of all adults. Less than half of children with ADHD ever "out-grow" it in adolescence or adulthood. If untreated, the disorder can have long-term adverse effects into adolescence and adulthood, so we always recommend early and aggressive treatment.

The disorder has different "looks" or "types." For some it severely impacts behavior, and for others it greatly impacts learning. For the group in the middle, it just impacts their attention, focus, concentration, and getting the job done.

Different Types of ADHD in Children

ADHD research literature, recent books, and common sense, all point to the fact that there are different types, or styles, of ADHD. In the past we referred to Attention Deficit Disorder: Inattentive Type, or Impulsive/Hyperactive Type, or a Combined Type. Today the diagnostic differences are a bit less clear, but the reality doesn't change. Even though doctors use the term "ADHD" in every diagnosis, one person with ADHD may have a completely different set of symptoms from another with ADHD.

Some with ADHD are Inattentive, some are Impulsive, and some are Impulsive and Hyperactive. The different "styles" have to do with the different parts of the brain that are impacted by the disorder. There are some common symptoms that each type of ADHD has, and then there are some symptoms that are specific to that particular type of ADHD.

While our observations are based on clinical observations and experiences, they are backed up by recent brain research. For years we have illustrated that there are different types of ADHD to parents and teachers by using the characters of a great children's book : Winnie the Pooh and his friends in the Hundred Acre Wood.

Different Types of ADHD : a closer look

Winnie the Pooh Type ADHD describes those who are inattentive, distractible, and disorganized. They tend to be friendly, but live in a cloud of "brain fog".

Tigger Type ADHD describes those who are inattentive, and also impulsive and hyperactive, restless and bouncy - "because bouncin' is what Tiggers do best!"

Eeyore Type ADHD describes those who are inattentive, but also suffer with a chronic low-grade depression. About 25 percent of those with ADHD also suffer from depression, especially females.

Piglet Type ADHD describes those with ADHD who have trouble shifting attention from one thing to another, worry alot, and are easily startled.

Rabbit Type ADHD describes those who also have trouble shifting attention, but are also inflexible, and can be demanding and argumentative. Though this might describe many teenagers anyway, we are here concerned with children, teens, and adults who have this type of ADHD...

And then there is a Troubled Type ADHD for which there is no character in the Hundred Acre Wood. The Tazmanian Devil from Loonie Tunes is probably a better cartoon character to illustrate this type of ADHD. Those with this style of ADHD tend to be very irritable and perhaps aggressive. They are often impulsive in their aggression. They can be defiant and disobedient. And very commonly have other specific learning problems.

Tiggers Like to Bounce... Bouncin' is What Tiggers Do Best! We call this type of ADHD "Tigger Type." This type of ADHD reminds us of Tigger from the Winnie the Pooh stories. ADHD Brain : Impulsive-Hyperactive Type

This Impulsive-Hyperactive or "Tigger Type" ADHD is the result of UNDERACTIVITY in the Prefrontal Cortex, both when the brain is at rest, and when the brain is performing tasks that require concentration. It is this front part of the brain that helps us with self-control, inhibition, and paying attention to our situation.

This classic type of ADHD is most often seen in males, and is the type of ADHD most often referred to clinics for treatment.

ADHD : Inattentive Like Winnie the Pooh

Inattentive ADHD: Head in the Clouds - Just Like Winnie the Pooh

Winnie the Pooh is the classic picture of Inattentive ADHD. Although Pooh is very lovable and kind, he is also inattentive, sluggish, slow-moving, and at times quite unmotivated. He is a classic daydreamer. In other writings we have called this "Space Cadet" style ADHD. These are people that suffer from "brain fog" as they go through their day.

People with this type of ADHD are often described as being easily distracted and in a fog. They are just not paying attention to you, or to their work. They have short attention spans to tasks that are difficult, or boring. And the problem is that most work is either difficult or boring. They often daydream when others are talking to them. They are easily bored unless they are in a very entertaining or rewarding situation. They often lose or misplace things like homework, car keys, purses, or reading glasses. They often have trouble planning ahead, and since they cannot find their car keys, they are usually late for appointments.

It is pretty common that these students will spend two hours doing a twenty minute homework assignment because they are so easily distracted, and then fail to turn in the homework the next day because they have either lost it in their backpacks or have stuffed it into their desk and forget where they put it. This happens daily for many of these kids.

ADHD Brain : Inattentive Type ADHD

This type of ADHD is caused by the prefrontal cortex of the brain actually slowing down (instead of speeding up in activity) when placed under a work load like reading or doing homework. This part of the brain looks fairly normal when "at rest," but actually looks like it is starting to fall asleep when asked to "go to work" on a task like a math worksheet or history homework. This makes it very hard to pay attention to school work, get homework done, listen to the teacher, clean your room, and so on.

We have actually observed this dozens of times with subjects wired to an EEG so that we could observe their brainwave activity. When at rest, the brainwave activity is pretty normal. But once the subject is asked to read, or to do a math worksheet, the subject's brainwave activity begins to look like the subject is falling asleep - and often they did fall into a deep sleep where we had to ask the parents to come in to wake up the subject. This sure makes school hard for these students!

Winnie the Pooh style inattention is seen mostly in girls. It responds well to stimulants, such as ritalin and adderall, but other interventions work well also.

Over Focused and Rigid ADHD

Over-Focused ADHD: Rabbit Tends to His Garden... and don't bother him.

The least flexible character in all of the stories of Winnie the Pooh and Christopher Robin has got to be Rabbit. Oh, he can get a lot of things done, and he's the one character who will be prepared when winter comes, but he has a very hard time shifting from one activity to another. He is absolutely "task oriented" and is focused to whatever that task might be.

The person with "Over-Focused ADHD" is much the same. He has trouble shifting attention from one activity to another, and he frequently "gets stuck" in loops of negative thoughts. He can be obsessive, and very inflexible. He can also be oppositional and argumentative to parents.

He may be like a "bull dog" and not give up until he gets his way, or until his worn-out parents finally say, "yes," to his 100th request for something. His parents are often worn-out, worn-down, fed-up, and ready to break. Parenting a child like this is hard.

Someone with "Over-Focused ADHD" is like Rabbit in that he may worry a lot, even about things that don't really matter very much. He can be oppositional at times because of his worry or anxiety, or just because he wants his way. He may argue a lot about getting his way. In this way he may seem obsessive, or compulsive. He will typically have a hard time shifting from one activity to another, and may need a lot of warning ahead of time before being asked to make a change in what he's doing. He may always seem to demand his way in things.

ADHD Brain : Over Focused and Rigid

The cause of this type of ADHD is an over-active Anterior Cingulate Gyrus. This part of the brain is over-active whether at rest, or under a "work load." And, to make matters more complicated, when a "work load" is put on the brain, such as school work or a chore to be completed, there is the common ADHD symptom of decreased activity level in the Pre-Frontal Cortex as seen with the Inattentive or Winnie the Pooh type of ADHD.

Treatment Strategies for Over-Focused ADHD

In this type of ADHD, treatment with stimulants can actually make symptoms worse. So can over the counter treatment with too much L-Tyrosine to increase dopamine production. So be wise with your treatment choices.

ADHD and Depression : Eeyore and the Limbic System

"Thanks for Noticin' Me" says Eeyore...

He walks slowly. He looks sad. He doesn't accomplish much. He's just glad to be noticed. This is Eeyore, the stuffed donkey who is so often in need of his tail being pinned back on.

Chronic Low Grade Dysthymia and ADHD

Those with this type, or style of ADHD are often inattentive with a degree of brain fog, and they may also often complain of a chronic low-grade depression. They often seem negative, or apathetic about projects, and they seem to have low energy levels and little enthusiasm. They often just don't seem to care about things. There are often feelings of worthlessness, or helplessness, or hopelessness.

ADHD Brain : Limbic System and Depression

SPECT scans show that in this type of ADHD there is increased activity deep in the limbic system, in parts of the brain called the thalamus and hypothalamus, when the brain is "at rest." There is also a decreased level of activity in the underside of the pre-frontal cortex.

When the brain is placed under a work load, as during a homework assignment, nothing changes. The over-active limbic system remains over-active, and the under-active pre-frontal cortex remains under-active.

This type of ADHD looks very much like a combination of ADHD and Depression. Some have suggested that up to 25% of children with ADHD are also depressed or suffer from a mild depression called Dysthymic disorder.

So it is with some kids with ADHD. This style of ADHD is very similar to the Rabbit style, except that with "Piglet style" the child's mid-brain is so over-aroused that the child is hypervigilant and very easily startled. He may be talking all of the time, and is probably touching everything in the room. And, this child is nervous or worried, or anxious. He has trouble shifting attention from one activity to another, and he frequently "gets stuck" in loops of negative thoughts. He can be obsessive, and very inflexible.

Treatment Strategies for Over-Focused ADHD

In this type of ADHD some stimulants, and too much use of L-Tyrosine to increase dopamine production can actually make the problem of over-focus worse. So be careful.
